When you submit your completed application, you MUST include these additional documents. Without them, your application will not be processed. Documents must be submitted by all buyers/applicants.
Please provide copies (not originals) of the following documents:
1. Last three (3) most recent paycheck stubs for all household members 18 years of age or older.
2. If you are self-employed, provide a year-to-date profit and loss statement, a California income tax return for 2010, and a business license.
3. A No Income Statement, signed and notarized, for any household member over 18 years of age with no source of income, if applicable.
4. A complete copy of the last three (3) years’ federal income tax returns and W-2 forms pertaining to these income tax returns. If you have not filed a 2010 federal income tax return, please provide a copy of your extension of time to file United States income tax return and your 2010 W-2 forms.
5. Copy of last consecutive three (3) months savings account statements and six (6) months checking account statements for all assets showing current value, including all bank accounts, investment accounts, and retirement accounts on organization letterhead for all members in the household 18 years of age or older. Attach all pages, including the front and back as well as blank pages for each account.
6. Copy of last consecutive three (3) months statements and documents from all other sources of income of all household members listed on the application on organization letterhead (for example, alimony, child support, disability, retirement, social security).
7. Affidavit of U.S. Citizenship or Permanent Residence Status accompanied by copy of birth certificate, passport, certificate of naturalization, or permanent resident “green card.”
8. Driver License or State Identification Card.
9. Social Security Card (All applicants).
10. Information Release Authorization Form (original and signed).
11. First Time Homebuyer Certification Form (original and signed), if applicable.
12. Verification of Local Residence Form (original and signed) and support documents, if applicable.
13. Verification of Local Employment Form (original and signed) and support documents, if applicable.
14. Verification of Veterans Status Form (original and signed) and support documents, if applicable.
15. Gift Verification Form (original and signed), if applicable.
16. Request for Transcript of Tax Return (Form 4506-T) must be completed for all members in the household 18 years of age or older.
